Clinitouch is a remote patient monitoring app and allows your healthcare team to monitor your health at home. Your healthcare team will let you know if you need to use Clinitouch and will provide you with access details.
Clinitouch is easy to use, simply answer the health questions allocated by your healthcare team and enter vital sign readings relevant to your condition. Your healthcare team will review the information you submit and will reach out if anything needs attention.
The app enables you to:
• Easily answer health questions
• Take vital sign readings
• See your health trend charts
• Receive messages and video calls from your healthcare team
• Access educational resources to help you learn and better manage your condition
